CEREC restorations – a guide for patients

At Cvejanović Clinic, we use a standard method for a complete reconstruction of upper and lower jaws (Hollywood Smile procedure) which is a time-consuming process requiring multiple trials (you can look at the Hollywood Smile detailed guide for patients here).

However, for all minor prosthodontic procedures, like single crowns, inlays, onlays or bridges up to three pieces, we use a much faster and up-to-date crown manufacturing system called CAD/CAM CEREC, which can produce a single crown in 30 minutes, with the whole procedure taking only one day! CEREC system uses premade ceramic blocks which are reduced by milling into crowns using a specialized apparatus. The dimensions of the blocks present a limit to what can be accomplished, which means that this system is best utilized for smaller prosthetic restorations.

There are five steps in creating dental crowns using the CEREC machine:

1. EXAMINATION / SENDING IMAGES

In order to get one or more crowns, a patient, first and foremost, needs to possess teeth unaffected by any degenerative process to place artificial crowns onto. Secondly, if these crowns are to be placed in the front region, they need to be identical in appearance to the adjacent teeth, and our doctors will present possible options for this. These are the only two conditions for completing the procedure, indeed, in one day, along with sending the digital pantomogram ahead of the date, with a few photos of teeth, all of which you can deliver personally.

If a patient comes in person to deliver pantomogram and photos, but wishing to have the crowns placed the same day, we need to arrange a date in advance, so we can have enough time for fabricating the pieces. We schedule the procedure only if a deposit is made. If the examination is done online, the deposit is also sent online.

2. CONTOURING

Contouring consumes the most time in the procedure and takes about 30 minutes per tooth . This is the only part of the procedure that is somewhat unpleasant, which is why a local anesthetic is used. Contouring is used to shape the tooth so that it can receive the crown in such way that it will result in a perfect match.

Contouring preserves the inner part and the root of a tooth, whereas the outer part, which was the cause of the patient’s dissatisfactions, is replaced by a zirconia crown. Patients have to understand that they are always going to wear artificial crowns from the first time their tooth has been contoured, but these crowns may last over 10 years.

Our great experience and advanced techniques of fabricating crowns enable us to place crowns even on teeth which were misaligned, making these new crowns perfectly aligned with the natural ones around them.

In case a patient would like crowns placed in positions of crooked teeth, they do not have to wear braces to correct the positions, since their problem is solved by placing new crowns.

3. 3D CAD/CAM CEREC IMAGING SYSTEM

CEREC (Ceramic Reconstruction) system consists of four components: a 3D imaging system, then, software which renders possible solutions for crown appearance, a milling and grinding unit, as well as a sintering, glazing and crystallizing furnace. It is a unique system enabling a 3D rendered, machine fabricated crowns, superior in precision compared to manual, impression-based fabrication.

Scanning of teeth is performed using a miniature camera which renders a 3D model of your jaw, and takes no more than a minute to complete. Scanned teeth have already been contoured, so that the software may create crown models which will sit perfectly on the contoured teeth and form the outer shape of the crowns. The appearance of the crown is planned using the advanced CEREC software technology, which helps create nature identical artificial teeth, not only as a single piece, but also as a string of multiple crowns aligned properly in your jaw.

The shade of the crowns is chosen jointly by the dentist and the patient in the beginning, since CEREC utilizes prefabricated and pre-shaded zirconia blocks. Later, we may slightly adjust the chosen shade when the crowns go into furnace.

4. CROWN FABRICATION

Once our dentists have adjusted and approved the shape and shade of a crown modelled after a 3D scan, the only thing left to do is fabrication. With incredible speed, using a robotic arm and continual water cooling, the CEREC milling machine turns a zirconia block into a perfectly shaped dental crown within minutes The milling machine is chairside, and the chamber where the shaping takes place is transparent, so that the patient may observe the creation of their new crown.

The blocks are made of robust, all-ceramic material and are suitable for up to three-piece bridges, and if we go over this number, the crowns become more easily damaged. The blocks are available in various sizes and shades.

Once a finished crown is taken out of the machine, it is inspected and entered into an apparatus where it receives its final strength, polish and shine. After cooling, the crown is ready to try on immediately. In all the years we have been using CEREC system, NOT ONE crown was found to be flawed.

5. PLACING CROWNS

Crowns made in CEREC machine are cemented onto teeth, much like crowns on natural teeth, whereas CEREC crowns which are placed on implants may be screwed in. Cementing is a swift process, lasting less than 30 minutes. . First, the crowns are placed on teeth without cementing, so that the patient can try them on and take a look at their appearance and shade, and once the patient and our team determine that no further corrections are required, the crowns are cemented.

After cementing, it is recommended to come for a checkup the next day, to rectify possible issues with the bite. After this, it is desirable to have regular checkups every 3 to 6 months, in order to prolong the service life of the crowns. In case a crown gets damaged within the warranty period of 5 years, our patient will get a new crown free of charge.

Immediately after cementing, the patient leaves our office and may resume everyday routines, that is to say, only 4 to 6 hours after first entering our office with the pantomogram!
