If you have decided to go for ALL ON 6 implant, rest assured that our dentists will perform the procedure according to the highest standards and that you will receive an exceptional permanent implant restoration, but part of the responsibility also lies with the patient, who must be ready to follow certain rules knowing that they are in for a bit of discomfort in the first period after getting implants.
In this article, our topic is exactly this period between getting the implants and getting the permanent restoration, which lasts at least three months.
Although the principle of placing implants is standard, the body's adaptation to ALL ON 6 implant takes longer and is more tumultuous than with smaller implant restorations, such as Hollywood smile with additional implants. While with minor restorations mild pain and minor swelling stop in a day or two at most, adaptation to ALL ON 6 takes three to five days. As the reactions fade away, the temporary implant will have already been finished and ready for placement.
Reactions vary from patient to patient, and mostly depend on the degree of tolerance to pain. Many patients do not want to take painkillers because they say they can barely feel any pain.
During the first 24 hours, there may be slight bleeding. Minor bruising or skin discoloration may also occur. In any case, the tissue around the implant should not be in any way agitated (or washed) during the first 24 hours. Smoking is strictly prohibited. When it comes to swelling, it is going to be present to a lesser or greater extent, where the swelling can be more or less visible, more or less unpleasant, but never as excessive as with inflammatory processes caused by tooth decay, i.e. bacteria.
Recovery from ALL ON 4 restoration is not necessarily faster than recovery from ALL ON 6 restoration, because although fewer implants are placed, the jaw of ALL ON 4 patients is more sensitive, so inflammatory reactions may occur.
Swelling after getting ALL ON 6 restoration is normal and expected. Swelling does not occur immediately after implant placement, but 2-3 days later, because during the first 48-72 hours, the drugs that the patient received before the procedure still prevent swelling.
It is very important that during the first two days after the procedure the patient uses cold compresses, and when the compresses are combined with anti-inflammatory drugs, there will be no major swelling, and the overall recovery will be easy and fast. The pain and swelling is expected to completely disappear within 5-6 days after the procedure, and for many patients even earlier.
The appearance of swelling and the speed with which it subsides depend to a large extent on the patient's general health. In younger patients, who do not have chronic diseases, it may happen that the swelling will not even occur.
Newly installed implants for ALL ON 6 work
Since a patient may be slightly disoriented after the procedure due to anesthesia, the patient receives all instructions regarding taking pain and swelling medication in writing, and additional instructions, depending on the patient’s condition, are also given during the mandatory check-up the day after receiving the implant. It is necessary to bring a new pantomogram image to this check-up, in order to show the position of the implant in the jaw after placement.

When leaving the office, we instruct patients on how to maintain hygiene until receiving a temporary implant and how to use the kit they have just received. In the period between getting the implants and getting the temporary restoration, patients mostly have to eat pureed food. Since the patient needs to have this diet for 2-3 days, ready-made oatmeal or biscuit with milk is not recommended, since they are not fully nutritious and contain too much sugar. It is far better to prepare meals from blended fruits, nuts and vegetables. If the patient so desires, we can also train them to use the Waterpik device, which is ideal for maintaining the hygiene of ALL ON 6 implant (we also recommend this device for natural teeth). Patient may start using the Waterpik device two weeks after the placement of the implant.
Patients spend two whole days without teeth, during which they stay in one of our apartments, or in their own flat, if they are from Belgrade and want to go home. Each patient gets their own apartment, but couples can still be accommodated in the same apartment. If a patient is interested in staying in our accommodation, they should state that during the initial appointment.
It is best that the patients are accompanied while leaving for home, and they must not drive themselves (because they have been given sedatives). On the other hand, patients who are not from Belgrade usually decide to stay in one of the two apartments above the office, which they can use for free for five days. When patients spend extra days in the apartment after the procedure, our team can administer the therapy even more efficiently thus shortening the adaptation period, as well as speed up the recovery from swelling and pain.
Temporary restoration is mounted three days after the implant is placed. Before receiving the temporary restoration, patient can also request to have the remaining sutures (stitches) shortened by laser in the places where the tissue was sewn after the placement of the implant. The sutures are resorbable which means they usually do not need to be removed, however if the patient feels discomfort, the remaining sutures can be removed after 14 days.

However, while the subsequently placed permanent implant stays in perfect position practically for life, with the temporary implant, during the three months of wearing it, gaps will gradually form between the gums and the restoration.
For this reason, it is extremely important that hygiene is properly maintained, because poor hygiene in this period poses the greatest risk to both the implant and the patient. Hygiene is best maintained with a Waterpik device and a toothbrush. Another very useful device is an ultrasonic toothbrush, which is far more efficient in removing deposits than either the regular and the electric toothbrush.
A portion of patients from abroad cannot come back to Serbia in order to get the permanent implant exactly three months after the placement of temporary restoration. They usually wear the work a bit longer, and, technically speaking, it can be worn for more than a year.
Apart from the fact that the permanent restoration is much more attractive – it will not turn yellow and is more firmly attached to the jaw than the temporary one – one of the reasons why it is desirable to get the permanent implant as soon as possible is that it makes the hygiene much easier to maintain. Since temporary implant is made of plastic, it is much easier for food deposits and traces of drinks and tobacco to stick to it, so patients who wear it for more than three months have to occasionally unscrew the implant themselves, take it off, and wash it.
The fabrication of the permanent implant begins towards the end of the third month of wearing the temporary implant, because three months is the shortest period during which the implants can be integrated into the jaw. As soon as the pantomogram scan confirms that the implants are ready to withstand the load of the permanent restoration, which is more massive than the temporary one, the final ALL ON 6 piece is received.
Patients from abroad can send a video by email after three months, and many decide to wait another month or two, since the temporary implant is suitable for everyday life during the first six months of wearing it, so there is no need to rush. For some patients it is necessary to wait for more than three months for complete osseointegration, but this does not exceed six months.
